Rebate forms are available online but offer must be redeemed by mail. Please refer to your Rebate form for more detailed instructions on submission.
Yes, we will accept a photo copy of pages 1 and 2 of your billing statement as your proof of activation as stated on your Rebate form.
You can request a copy of your billing statement by contacting your service provider.
If the Rebate Form is submitted or postmarked after time specified, you will not qualify and the offer will be void.
Rebate will be mailed within 10 weeks from the date of properly completing the Rebate submission requirements.
While it is not required, an email address will allow us to provide you with up-to-date information regarding your Rebate submission and tracking information.
You can check the status of your Rebate by clicking on Track Rebate and completing the requested information.
